About This Form

This intake form is entirely optional—but meaningful planning depends on understanding what matters to you. The more insight you’re able to share, the better I can tailor recommendations, anticipate needs, and focus our first consultation on what truly moves you.

You do not need to have every answer figured out. If some questions feel unclear, incomplete, or simply premature, feel free to leave them partially answered or blank. We can explore those areas together when the time comes. This form is a starting point for conversation, not a test—and uncertainty is often the beginning of good travel planning.

For certain highly tailored or research-intensive itineraries, a $250 consultation fee may apply; if so, I will always discuss this with you in advance, before investing significant time, so expectations are clear and aligned from the outset.
Who will be traveling, and what is your relationship to one another? (For example: solo traveler, couple, family with ages, friends, multigenerational group, academic or special‑interest group) How would you describe yourself as a traveler? (What excites you most about travel? What tends to frustrate you?) What past trip best represents the kind of travel you love—and why?
What is driving this trip right now? (Celebration, rest, learning, adventure, connection, milestone, curiosity, obligation, etc.) If this trip were a success, what would you say afterward that makes it feel worthwhile? Are there any themes or interests you would like this trip to center on? (Examples: history, food & wine, cruising, beaches, culture, nature, sports, genealogy, faith, espionage/Cold War history, WWII, wellness, accessibility, etc.)
Are there destinations you are drawn to right now? What attracts you to them? Are there destinations you definitely want to avoid? Why? How open are you to alternatives or nearby options if they better meet your goals?
How would you describe your ideal daily pace while traveling? (Slow and immersive, moderately active, fast‑paced and full, or a mix) What balance do you prefer between structured activities and free time? How do you feel about guided experiences versus exploring independently?
What matters most to you in accommodations? (Location, comfort, luxury, character, brand familiarity, accessibility, value, quiet, views, etc.) How do you feel about different modes of travel? (Cruising, trains, flights, road trips, ferries—likes and dislikes) Are there any comfort, mobility, or accessibility considerations we should plan for?
What types of experiences energize you while traveling? What experiences do you prefer to limit or skip altogether? Are there any “must‑do” moments you already have in mind?
How important is food and dining to your travel experience? (Street food, fine dining, local specialties, familiar options, dietary needs) How do you like to engage with local culture? (Museums, neighborhoods, performances, lectures, local guides, casual exploration)
What travel dates are you considering, and how flexible are they? Are there external constraints we should be aware of? (School calendars, work schedules, events, weather preferences)
How do you generally think about travel budgets? (Value‑focused, comfort‑first, experience‑driven, luxury‑leaning, cost‑conscious) Are there areas you prefer to prioritize spending—or save? Is there a budget range you are comfortable discussing during our consult?
How involved do you want to be in the planning process? What has (or hasn’t) worked for you with past travel planning—agents or DIY? How do you prefer to receive recommendations and information? (Conversation, written briefs, options to compare, visuals, checklists)
What concerns you most when traveling? (Logistics, safety, health, uncertainty, disruptions, accessibility, crowds) What helps you feel confident and well‑supported before and during a trip?
